The question of evaluating the effect of a medical test on subsequent patient outcomes has proved to be particularly challenging. Insofar as a test provides information, which is subsequently incorporated into further diagnostic and therapeutic decision making, its impact is strongly affected by the intervening decisions about medical care and ultimately by the effectiveness of available therapy.  In this presentation, after reviewing the problem and the classical solutions we will focus on recent approaches to the study of the impact of tests, which include the analysis of data from large secondary databases and registries and data from randomized studies with adjustments for confounding.
